Brow Lamination Consent Form: What to Include (Free Template)
What a brow lamination consent and intake form should cover, from retinoids and recent tints to dye allergies, patch tests and a signed release, with a free printable template.
Why brow lamination needs a consent form
Brow lamination uses perm-style solutions to soften and reset the hair, and most services add a tint. Both can irritate skin near the eyes or trigger a reaction in clients with dye allergies. A signed consent and intake form shows the client disclosed their skincare, health and allergies, had a patch test where needed, understood the normal risks and agreed to follow aftercare. It protects your business if a reaction or complaint comes up, and many insurers expect one on file.

Record every visit
Note the lift and setting times, tint shade and how the brows responded on a service record, so you can repeat what worked or go gentler next time. Ask about new skincare and medications at every appointment. This guide is general information, not legal or medical advice, so check the rules where you work.
