Lash and Brow Tint Consent Form: What to Include (Free Template)
What a lash and brow tint consent form should cover, from PPD and hair dye allergies to contact lenses, eye conditions, patch tests and a signed release, with a free printable template.
Why tinting needs a consent form
Lash and brow tinting is quick, cheap and one of the easiest services to add to a menu, which is exactly why it gets treated casually. But you are applying a dye and a developer a few millimetres from the eye. Many tints contain PPD or similar dye ingredients, the same family of chemicals behind most hair dye allergies, and a reaction around the eyes can mean swelling, redness and a very unhappy client.
A signed consent and intake form shows that the client told you about allergies, eye problems and recent treatments before you started, that they understood the normal risks, and that they agreed to follow aftercare. It helps you decide whether to go ahead, patch test first or refer them elsewhere, and it gives you a record if a complaint or insurance question comes up later.

- Client details: name, phone, email, date of birth and how they found you
- Dye allergies: any reaction to hair dye, PPD, black henna tattoos or a previous lash or brow tint
- Eye conditions: conjunctivitis, styes, blepharitis, or very dry or watery eyes, which usually mean waiting until they clear
- Recent eye surgery or injury: LASIK, cataract surgery or trauma in the last few months
- Contact lenses, with a reminder to remove them before the tint
- Skin around the eyes and brows: eczema, psoriasis, cuts or irritation
- Medications and recent treatments: retinoids, Accutane, and brow waxing, peels or laser in the last 48 hours
- Pregnancy, nursing or medical treatment such as chemotherapy, which can change how hair takes color
- Patch test status: done, declined, or a repeat client with no changes
- Service and color: lash tint, brow tint, both, or a tint added to a lift or lamination, plus the shade wanted
- Aftercare acknowledgment and a plain-language release with the client's signature and date
The patch test question
Most tint manufacturers recommend a patch test 24 to 48 hours before the first appointment, and many insurers expect one too. Put a small amount of the mixed tint behind the ear or on the inner elbow, ask the client to leave it alone, and check for itching, redness or swelling before you book the full service. If there is any reaction, do not tint.
Some clients will refuse a patch test because they want the service today. Your form should have a clear box for that, and the consent wording should say that a client who declines accepts the added risk. Re-check allergies at every visit, and repeat the patch test if the client has started new medication, had a reaction anywhere else, or has not been in for six months or more.
Aftercare to put in writing
Ask clients to keep lashes and brows dry for 24 hours and to avoid steam, saunas, swimming, heavy sweating and rubbing their eyes. For the next few days they should skip oil-based makeup removers, scrubs and other brow treatments on the area, since these fade tint faster. Tell them to contact you straight away if they notice irritation, and have them initial that they received the instructions.
Record every service
A simple service record makes every repeat visit faster. Log the date, service, patch test date, tint color, developer strength, processing time and any notes on how the color took. Lashes and brows often need different timings, and natural hair color changes the result, so writing down what worked saves guesswork next time and makes it easy to spot a past reaction before you start.
