Lash Extension Client Intake Form: What to Ask Before a Full Set
The questions every lash artist should ask on a client intake form before the first set, and how to keep records.
Why intake comes before consent
An intake form collects the facts you need to decide whether a client is a good fit for extensions today. The consent form then records that they understood the risks. You want both on file.
