Lash Extension Consent Form: What to Include (Free Template)
What an eyelash extension consent form should cover, from adhesive allergies and eye health to patch tests and a signed release, with a free printable template.
Why lash artists need a consent form
Eyelash extensions are applied millimeters from the eye with a cyanoacrylate adhesive. A signed consent form shows the client was told about the normal risks, shared their health and allergy history and agreed to follow aftercare. It protects your business if a reaction or complaint comes up, and many insurers expect one on file.

- Conditions that affect retention: thyroid issues, chemotherapy, hair loss conditions, pregnancy
- Comfort: whether the client can lie still with eyes closed for up to two hours
- Patch test status: done, scheduled or declined
- Risks in plain words: irritation, redness, an allergic reaction and normal shedding
- Signature and date
Patch tests and records
Offer a patch test 24 to 48 hours before a first full set, especially for clients with sensitive skin or past reactions, and note the result. After every set and fill, log the curl, thickness, lengths and any redness or itching. If a client starts reacting months later, your records show what changed. This guide is general information, not legal advice, so check the rules where you work.
