Lash Lift Consent Form: What to Include (Free Template)
What a lash lift and tint consent form should cover, from contact lenses and recent eye surgery to dye allergies, patch tests and a signed release, with a free printable template.
Why lash lifts need a consent form
A lash lift uses perm-style solutions right next to the eye, and most appointments add a tint. Both can sting, irritate the eye or trigger a reaction in clients with dye allergies. A signed consent and intake form shows the client disclosed their eye health, skincare and allergies, had a patch test where needed, understood the normal risks and agreed to follow aftercare. It protects your business if a reaction or complaint comes up, and many insurers expect one on file.

- Client details: name, phone, email and date of birth
- Contact lenses and eye surgery: lenses must come out, and LASIK or other surgery in the last 6 months is a reason to wait
- Recent lash services: lifts, perms, tints or extensions in the last 6 weeks
- Eye conditions: conjunctivitis, styes, blepharitis, dry or watery eyes
- Retinoids and skin: Retin-A, retinol, Accutane, eczema or broken skin near the eyes
- Allergies: hair dye and PPD, perm solutions, adhesives and fragrance
- Patch test: whether one was done and the date
- Services and desired curl: lift, tint or both, natural or dramatic
- Risks in plain words plus a signature and date
Record every visit
Note the rod size, solution timings and tint shade on a service record, so you can repeat a great curl or go gentler next time. Remind clients to keep lashes dry for 24 to 48 hours and book their next lift in 6 to 8 weeks. This guide is general information, not legal or medical advice, so check the rules where you work.
