Why microblading needs a consent form

Microblading is a semi-permanent cosmetic tattoo, so it breaks the skin and carries real risks: swelling, scabbing, infection, allergic reactions and healed color that fades or shifts. A signed consent and intake form shows the client disclosed their medications, health history and allergies, approved the brow shape and pigment before you started, and agreed to follow aftercare. It protects your business if a complaint comes up, and most insurers and many local health departments expect one on file.

What to include

Record every session

Log the blade or needle, pigment, numbing used and how the skin healed on a client service record, so the 6 to 8 week touch-up matches the first session. Take before and after photos at every visit. This guide is general information, not legal or medical advice, so check the licensing and consent rules where you work.
