Nail Client Intake Form: What to Ask Every New Client
What a nail client intake form should include, from allergy and health questions to shape preferences and consent, with a free printable template.
Why every nail tech needs an intake form
An intake form protects you and your client. It flags allergies and health conditions before you open a product, records what the client wants, and gives you a signed record if a reaction or complaint ever comes up. It also makes a first visit feel professional from the moment the client sits down.

- Contact details: name, phone, email, date of birth and how they found you
- Allergies: acrylic monomer, gel, latex, fragrance and adhesives
- Health: diabetes or circulation problems, skin conditions, pregnancy, blood thinners
- Recent injuries: cuts, infections or damaged nails on the hands or feet
- Nail history: natural, gel polish, acrylic, builder gel, dip or press-ons
- Preferences: shape, length, colors and inspiration
- Consent: a short statement and signature confirming the information is accurate
Keep a service record too
After the intake, log each visit: date, service, products and colors, shape and length, price and notes. When a client asks for 'the same as last time', you have it. Noting redness or lifting after each visit also helps you spot a sensitivity early. This page is general information, not legal or medical advice.
