Spray Tan Consent Form: What to Include (Free Template)
What a spray tan consent form should cover, from DHA and fragrance allergies to asthma, pregnancy, recent waxing and a signed release, with a free printable template.
Why spray tans need a consent form
A spray tan feels low risk, but you are misting a DHA-based solution over a client's whole body, often in a small booth or room. Most clients never have a problem. A few react to DHA, fragrance or other ingredients, some have asthma that misting can set off, and others end up streaky or patchy because they waxed, shaved or exfoliated at the wrong time.
A signed consent and intake form shows the client told you about allergies, breathing problems and recent treatments before you started, understood the normal risks and agreed to follow aftercare. It helps you decide whether to go ahead or adjust the service, and it protects your business if a reaction or complaint comes up later. Many insurers expect tanning businesses to keep one on file for every client.

- Client details: name, phone, email, date of birth and how they found you
- Allergies: DHA, fragrance, nuts, latex, alcohol and past reactions to self-tanner
- Asthma or breathing problems, since misting can trigger them (offer a nose filter or skip the face)
- Pregnancy or nursing, so the client can check with their doctor first
- Skin conditions: open cuts, rashes, eczema, psoriasis or very dry, peeling skin
- Recent hair removal and treatments: waxing, shaving, laser or peels in the last 24 hours
- Sunburn and current retinoids or prescription skin medication
- Skin tone and shade wanted, so you can pick the right solution
- Service today: full body, rapid or express, contour, or an event or bridal tan
- Aftercare acknowledgment: when to shower, what to wear, and how to make the tan last
- Risks in plain words, including that a spray tan gives no sun protection, a release, and the client's signature and date
Aftercare to put in writing
Most complaints about a spray tan come from what happens in the first day. Ask clients to wait 8 to 12 hours before showering, avoid sweating, water and tight clothing until then, and rinse with lukewarm water only. For the next few days they should moisturize daily, skip scrubs, long baths, pools and hair removal, and pat dry instead of rubbing. Have them initial that they received these instructions so there is no confusion later.
Record every session
Use a client service record to log the date, service, solution and shade, how the tan developed and the price. Noting the shade that worked best makes repeat bookings faster, and checking the record before each visit makes it easy to spot a past reaction. For events and weddings, book the tan 1 to 2 days before so the color has time to develop.
