Waxing Consent Form: What to Include (Free Template)
What a waxing consent and intake form should cover, from retinoids and Accutane to skin conditions and a signed release, with a free printable template.
Why every waxing pro needs a consent form
Waxing removes a layer of skin cells along with the hair, so certain medications and skin treatments can turn a routine service into lifting, burns or bruising. A signed consent and intake form shows the client disclosed their skincare, medications and health, understood the normal risks and agreed to follow aftercare. It protects your business if a reaction or complaint comes up, and many insurers expect one on file.

Re-check at every visit
Skincare and prescriptions change between appointments, so ask about new products every time and note it on a service record with the date, areas, wax type and any reaction. For intimate services like Brazilian or bikini waxing, add a separate consent section. This guide is general information, not legal or medical advice, so check the rules where you work.
